• Copy of a signed lease/rental agreement, signed closing disclosure, settlement statement, purchase agreement, or a warranty deed
  • All documents must have all the required signatures and seals (both landlord and tenant signatures required)
• Photo Identification (Govt Issued)
  • Expired identifications will not be accepted

The rental owner reactivation form below is for landlords only. This form can ONLY be filled out in addition to the application above. It will allow the landlord to have their services reactivated automatically when their renters terminate their services.
