The Seminole County Fire Department is proud to provide its Community Paramedicine Program to serve its residents.

Community Paramedicine is a proven, proactive approach and solution to offer quality care and preventative services. The Program utilizes paramedics to provide guidance and care while helping residents become more independent from the emergency system and to use current available resources. All of this is provided at no cost to the resident.

The Program integrates specially trained prehospital professionals, who assist with closing healthcare gaps and improving access while working to decrease residents’ overall healthcare costs.

Community Paramedic Program: Target Patient Populations

  • High Emergency Room (ER) System Utilizers — designed to help patients who are high utilizers of the 911 system/ERs. The program provides education and connects the patient with resources available in the community.

  • Chronic Mental Health Issues or Problems with Substance Abuse — identifying patients who would be better served outside the emergency room setting and redirecting these individuals to the appropriate community resource(s) and helping with proper medication management.

  • Diabetic Patients — providing nutritional and disease management education to diabetic patients to prevent unnecessary Emergency Department / EMS utilization.

  • Frequent Falls — helping mitigate fall risks by performing in-home safety assessments and fall prevention education.

  • Congestive Heart Failure / Chronic Respiratory Conditions — helping manage medication compliance, nutrition, and education for patients who are diagnosed with congestive heart failure or other chronic respiratory conditions.
